Dawn led on the discharge of a patient who was on her ward in Cefn Coed Hospital to a nursing home in Scotland. This was a complicated discharge and Dawn had to work with the clinical team, family, providers in Scotland, CHC funders locally, NHS Scotland in order to secure a placement.

The family dynamics involved meant that Dawn had to work sensitively and closely with the family.

The discharge planning and indeed eventual transfer involved a great deal of time, effort and dedication on Dawn’s part and she truly did go the extra mile.

The result was that the patient was eventually moved to Scotland to be close to his family, including his wife and the family duly expressed thanks and gratitude to Dawn in particular and her team.
